This gives me a sinking feeling and I have to ask, what did they do ?

Firstly, that sinking feeling is fear of the unknown and worry this could somehow pertain to your situation. It doesn’t. It’s nice to know that we all go through many of the same experiences and have the opportunity to gain perspective on others’ similar stories. What is unhealthy IMO is (even if your person is here) letting yourself think any letter is to, for or about you/your person. Key words mean nothing. It just leads to more misunderstandings. So unless your person said, “Hey, I’m too cowardly to say this to your face so here is my letter entitled blah blah blah” then don’t assume.

P.s. I have no problem telling (him) my feelings in the real world. Anything like this was already said to his face and I’m just sporatically venting out to the universe.

I understand you. Maybe we have to go through these experiences in order to grow. Maybe it will get easier with time, maybe we will only be numb enough with time. Maybe not.

I can also relate to what you commented. There was a time when I assumed that every letter was from them and directed to me…

I wish you that you find enduring happiness, author!
